Any injectable substance administered during the course of an operation may induce an immediate hypersensitivity reaction, which could be fatal. Preoperative and postoperative investigations must be done to identify the mechanism of the reaction and the responsible agent. This includes assaying the level of histamine, tryptase and specific IgE at the moment of the reaction, as well as doing skin tests six weeks later. Exploring the possibility of a cross-reaction should be done systematically in case of allergy to curate in order to establish precise recommendations in case of the need for future anesthesia. Basophil activation tests, notably by flow cytometry, applied to the diagnosis of sensitization to a drug allergen and exploration of cross-reactivity, are undergoing rapid development.
